Adds a durable, redacted operator incident ledger, private Slack alert delivery with receipts, run and backend error correlation, discrepancy detection for declared inability, and an admin-only paginated incident API. Validation: typecheck, ESLint, oxlint, changed-file Prettier, 8 incident tests, 29 config tests, affected route/catalog tests, and diff check pass. The repository-wide Windows test runner still has a pre-existing supervisor fixture hang; the affected tests were isolated and passed.
