Align Gallery/MIGXdb/GridClassKey examples with 3.x bindings, fix stale setting defaults and removed-setting pages, and drop manager/min troubleshooting that no longer applies. Refs modxorg#121.
Sync the remaining changed-default pages (including resource_tree_node_name_fallback) with Revolution 3.x lexicon and defaults, and mark leftover removed-setting pages that still read as live.
