Converge adaptive effect outcomes without erasing useful answers

Outcome

Replace the opaque counts-only finality response with a bounded outcome report grounded in acknowledged effect receipts and exact current-state evidence. Useful model synthesis is retained as explicitly non-authoritative context whenever canonical facts must lead.

Root cause

Historical attempt statuses were being treated as the current represented state. An indeterminate create receipt could continue to veto the turn even after exact same-target evidence, and the finality layer then replaced the model's detailed partial-result explanation with generic status counts. Reconciliation observations were also not durably acknowledged because their phase was rejected.

User impact

• Partial paper-representation turns now name the represented result, correct actor scope, and the PDF acquisition failure code and cause.
• The original attempt remains historically honest while acknowledged exact observations describe current state.
• Wrong-target, multi-target, unacknowledged, and genuinely unresolved cases remain unresolved.
• Blind duplicate creates remain blocked, while exact absence can unlock the same safe retry and declared idempotent workflow recovery remains available.
• Bare typed workflow failure codes survive into episode telemetry.

Validation

• 173 passed — complete adaptive-turn service suite.
• 202 passed — turn-record journal, directly affected telemetry reads, durable workflow, termination telemetry, and response-route suites.
• Exact incident plus held-out wrong-target, multi-target, missing-acknowledgement, generic transport-error, redaction, and journal-ordering cases.
• Python compilation, Ruff E9,F, and diff-integrity checks.
• Rebased and rerun against current main after PR Add owner-scoped LinkedIn export MCP #388.
