feat: expose public AgentTeams bridge

Summary

• expose a versioned, read-only agentTeamsBridge Cordis service and ./bridge package export
• publish durable captain-scoped lifecycle projections with approval ordered before scheduler dispatch
• cover HMR disposal, immutable projections, subscriptions, failure isolation, archive reads, and real lifecycle paths
• prepare 0.1.16-rc.2 for the npm next release workflow

Release

The branch declares 0.1.16-rc.2 with publishConfig.tag=next. After review and merge, an upstream maintainer can create and push annotated tag v0.1.16-rc.2; the existing OIDC workflow will rebuild, run its host matrix, publish the verified candidate to npm next, and create the prerelease.
