Adds the reconciler-only root package transition trigger. It requires an exact active context, preserves metadata outside mcpGrantBlock, uses the reusable S3 canonical validator for both old/new markers, and permits only blocked marker transitions or canonical removal to ready. Validation: git diff --check and static coverage.
