The column-category.csv file repeated the same allowable categories for
many columns, most notably the cat/cont variable-type values across six
different columns.

Define each set of values once in a new category-set.csv and reference
it by name from a new categorySet column in column-metadata.csv, so each
category column is now fully described in a single row.
