Restructure INDEX into Start/Build/Decide tiers and demote vision docs. Refresh architecture, concepts, stack-position, and field-services for v0.3.4 shipped scope; fix stale v0.2 voice; ripple README and CONTRIBUTING.

Mid-pass public docs sweep (not the #66 capstone). Brings the doc tree in line with v0.3.4 shipped scope and separates onboarding path from vision/reference material.
