Implement #pragma once

I much prefer #pragma once over #include guards.  Any chance ucpp could be
modified to recognize this?  Any pointers on how I might get started doing
it myself?

ucpp is intended to support ISO C99 preprocessor or lexers.  #pragma once is 
currently 
non-standard.   You can start with sample.c, define HANDLE_PRAGMA in tune.h, 
and 
create the behavior for the #pragma once.

Think I got this working, at least in non-lexer mode, which is what I needed.  
It is
a bit of a hack and may not be robust enough to consider incorporating into your
tree, but if anyone is interested I can pass along the code.

Great.  I'd be happy to take a look at it.  At worst, we could make it a 
define'able option.
