Remove fork-me-in-github ribbon from all example pages

It adds noise and it's not necessary in every page - it should be enough with index.html having it

Only https://mozdevs.github.io/MediaRecorder-examples/download-blob.html and https://mozdevs.github.io/MediaRecorder-examples/media-recorder-mimetypes.html still have the ribbon
